Please fill out the following application form to help us with the adoption matching process. To fill out this form, save this info from the “About Me” section of their adoption profile:
- Animal name
- ID number
- Location
Next, complete and submit the appropriate application form below.
Be sure to select the application link for the animal centre where the animal you are interested in is located. Applicants must be 19 years of age or older.
Please note: We often receive multiple applications for the same animal. Due to volume, we are only able to respond directly to the first few applicants who may be a match for the animal. We appreciate your patience and understanding – if others submit an application for a certain animal before you, we hope you will keep searching for a pet on our website as new animals are taken into care daily.
